Our Financial Policy

Thank you for choosing us as your health care provider. We are committed to providing you with the best possible care, and we are please to discuss our professional fees with you at anytime. Your clear understanding of our Financial Policy is important to our professional relationship. Please ask if you have any questions about our fees, Financial Policy or your responsibility. All patients must complete our Patient Information Form before seeing the doctor. If you do not have insurance, full payment will be due at conclusion of your first visit. You may pay by cash, check, Visa, Master Card, or American Express.

Regarding Insurance

We may accept assignment of insurance benefits. We, however, require your co-pay on the bill to be paid at the conclusion of your visit. While we are happy to bill your insurance company for you, this is a courtesy extended by our office. It is your responsibility to insure that our office has up to date insurance information. Any bill that has not been processed by your insurance company within 120 days will become your responsibility. Also please note that in those cases where your insurance deems a service or procedure as a “not covered service,” the responsibility for the bill will be yours. These services will not be subject to usual/customary or contractual obligation modifications. We will be happy to bill your secondary insurance as well; however, billing any insurance coverage beyond the secondary will be the patient’s responsibility. If we are not a provider with your insurance company, the usual/customary or provider disount deductions will not apply, and you will be responsible for the full charges. Any surgical or obstetrical co-payment balances are to be paid in full within 90 days after the date of treatment.
